EMERGENCY SOLUTIONS GRANT REQUEST FOR PROPOSAL

The City of Toledo Department of Neighborhoods and the Toledo Lucas County Homelessness Board (TLCHB) are requesting proposals from qualified applicants for a case manager. The position will primarily provide case management services to unaccompanied adults who have completed a Service Prioritization Decision Assistance Tool (SPDAT) assessment and qualify for rapid re-housing.
The proposal will be funded under the U.S. Department of Housing and Urban Development’s Emergency Solutions Grant (ESG) program. The ESG program provides funding to (1) engage homeless individuals and families living on the street; (2) improve the number and quality of emergency shelters for homeless individuals and families; (3) help operate these shelters; (4) provide essential services to shelter residents, (5) rapidly re-house homeless individuals and families, and (6) prevent families/individuals from becoming homeless.
